When patients ring clinics, hour by hour across 172,060 answered calls, and what the pattern means for front desk cover.
Most clinics plan their phone cover around a familiar picture: a busy morning, a quiet lunch, a steadier afternoon. The data matches that picture everywhere except lunch.
This post sets out when patients ring, hour by hour, across 172,060 calls answered for around 500 clinics on the BookedSolid platform, covering every call since the platform launched in January 2025. The aim is to show the shape of the whole day, so a practice can plan its phone cover around it.

Lunchtime Is the Third-Busiest Hour
Across the 172,060 calls, 12pm is the third-busiest hour of the day, with 17,764 calls. Only 9am and 10am are busier.
The midday hour sits level with 11am, which took 17,687 calls, and ahead of every hour of the afternoon. There is no lunchtime dip: even 1pm, with 15,288 calls, is busier than any hour from 2pm onwards.
About one in five calls (19%) arrive between 12pm and 2pm. That is the window when many front desks are at their thinnest, with teams breaking for lunch in turns or the desk closed while everyone eats.
The guide to after-hours call answering already counts lunch breaks as after-hours time. The hourly data backs that up.
The Morning Rush
9am is the busiest hour for clinic calls, taking 20,414 of the 172,060 calls, almost one in eight. The three hours from 8am to 11am bring in nearly a third of the day's calls between them (31%).
The day starts fast. The 8am hour alone takes 13,072 calls, more than any hour from 4pm onwards.
The dataset is hourly, so it does not split calls by day of the week. In BookedSolid's experience, though, Monday is the heaviest day of the week, and Monday at 9am is the peak of the clinic week.
After Closing Time
Calls rise again once the working day ends. The 7pm hour takes 4,658 calls, 15% more than the 4,059 taken at 6pm.
More than one call in four arrives when the clinic is closed. Across the dataset, 26% of calls are out of hours, measured against each clinic's own opening hours and including weekends. That figure treats lunch breaks as in hours, so the midday rush sits on top of it. Bookings follow the same pattern: across the platform, 32% of appointments are booked out of hours.
Overnight is quiet. Only about one in 200 calls (0.5%) arrive between midnight and 6am, so the small hours can wait for the morning.
The missed calls and after-hours enquiries guide sets out what each unanswered call costs a clinic.
What Callers Want
Two in five calls (40%) are booking related: the caller wants to make, cancel or move an appointment.
On average, two thirds of calls are handled from start to finish by the AI that answers them, with no one at the clinic stepping in.
What This Means for Your Front Desk
Four practical steps follow from the data.
- Cover 12pm to 2pm on purpose. Stagger lunch breaks so someone owns the phones through the middle of the day, rather than everyone breaking at once. One in five calls arrives in that window.
- Keep lunch off voicemail. A line that diverts at midday is unattended during the third-busiest hour of the day. If nobody can pick up, choose cover that can take a booking rather than a message.
- Plan for Monday at 9am. 9am is the busiest hour in the data and Monday the heaviest day in BookedSolid's experience, so rotas and callback lists should expect a heavy Monday morning.
- Make evening cover a decision. Calls rise at 7pm, so decide how they are answered: a person on a late rota, a callback arrangement, or an AI receptionist. Overnight can wait until morning.
Cover in place for the hours patients call

Frequently Asked Questions
What is the busiest time to call a clinic?
The busiest hour is 9am, which takes almost one in eight of the 172,060 clinic calls in BookedSolid's dataset. The morning from 8am to 11am carries nearly a third of the day's calls. Volumes then ease through the afternoon, with every hour from 2pm onwards below 15,000 calls.
Is lunchtime busy for clinic phone lines?
Lunchtime is one of the busiest periods of the day. 12pm is the third-busiest hour, level with 11am and ahead of every afternoon hour, and about one in five calls arrive between 12pm and 2pm.
What is the busiest day of the week for clinic calls?
In BookedSolid's experience, Monday. The dataset behind this post is hourly rather than day by day, so it does not give each day's share, but across the clinics the platform answers for, Monday is consistently the heaviest day. Combined with the 9am peak, Monday morning is the time to staff for.
How many clinic calls arrive out of hours?
More than one call in four arrives out of hours. In BookedSolid's dataset of 172,060 calls, 26% arrive outside each clinic's own opening and closing hours, with weekends included. Lunch breaks count as in hours.
